Organização de prontuários: um problema territorial na Equipe de Saúde da Família do Jardim dos Pescadores em Três Marias, Minas Gerais

The disorganization of the charts in the Family Health Program team Jardim dos Pescadores main causing many detached charts, due to the lack of registration of families in our area. At least 2,500 people live in the area covered by the Jardim dos Pescadores, but there are only patients 900 registered, generating 1600 detached charts. The difficulty in registering these families is due to the large extent of our territory, which covers rural areas as far as 40km from the Basic Unit of Health, and the lack of transportation for the team to sign the families that already addressed in the unit even without prior registration. The chart is a very important document, it contains not only the patient's medical history, but also allows us to estimate other characteristics, such as his attendance to health care and socioeconomic status. The lack of organization of Family Charts affect the quality of the interventions proposed by the staff, to make it impossible for them to view disease in the family context and the weaknesses and strengths of this family. The intervention plan was to register patients and group the detached charts on Family Charts. After registration of 89% of the population of the territory covered, as expected, the frequency of appointments without medical records decreased, there was optimization of the renewal of prescriptions from patients that were already in following and improvement in active search for patients in situations of social risk or inadequately controlled. There were secondary benefits such as increasing the number of first appointments.
