dc.description.abstractElevated blood pressure is an independent risk factor for cardiovascular disease. Hypertension has high medical costs, mainly due to its complications: cerebrovascular disease, coronary artery disease, heart failure, chronic renal failure and vascular disease. Several factors have been implicated in adherence to treatment of hypertension. Among them, we highlight the chronic and asymptomatic disease, the doctor-patient relationship, the complexity of treatment regimens and side effects of medications. Based on the complexity of care and clinical monitoring of these patients, it is currently recommended multiprofessional approach to the hypertensive patient. This approach is based on the quality of care and aims to motivate the hypertensive patients in searching the control of their disease. Life style modifications and also the adherence to anti-hypertensive medication are encouraged. The fact that high blood pressure is one of the main risk factors for developing ischemic heart disease and cerebrovascular disease and the second largest cause of death in our country, contributed to the development of this work, whose goal was to conduct a literature review to support the elaboration of a proposal for the hypertensive patients care. One of the strategies to be adopted include presentations in the waiting room, explanatory leaflets, municipal campaigns, making cards and card distribution of medicines, with the flexibility of direct approach towards clinical and social context of each patient. In this way, we seek to increase the effectiveness of treatment of hypertension through careful compartmentalized between different healthcare professionals. Furthermore, it is essential that the patient care is centered in their cultural, social and economic context.
