Introduction: Apical periodontitis is a disease originated by a pulp necrosis which is initiated by the entry of bacteria into the root canal that infect it, this access can occur due to several factors such as the presence of a maladaptive restoration and inadequate disinfection of the root canal. at the time of performing endodontic treatments among others. Cone beam computed tomography (CBCT) is a modified diagnostic aid of conventional tomography which uses an algorithm to create a three-dimensional image. Next, the sections corresponding to the approach of the problem are explained, where the importance of an early detection of apical periodontitis and its implications regarding the patient's systemic diseases is explained, as well as the need to organize the existence information regarding this topic since It is not performed. Objective: Systematize information on the prevalence of periodontitis through conventional cone beam tomography according to the evidence from the literature in observational studies. Methodology: A systematic review was carried out, being therefore a retrospective and secondary observational study. In this work, 11 articles were included in English, Spanish and Portuguese, extracted from the academic Google database, Pubmed, Scopus and the main of Web of science collection that reported the prevalence of apical periodontitis during the last 10 years. Results: Conclusions: A considerably high prevalence was reported in endodontically treated teeth compared to those that lack this treatment. The prevalence evaluated in tomographies obtained considerable values, which forces the professional to implement actions to decrease this value. The quality of the studies was in an acceptable range, which reduces the bias in this study.
