Heart failure is one of the most prevalent chronic diseases in the world, generating high mortality and morbidity for patients and high costs for the health system. In Colombia there are around 1.2 million people affected with this disease and only 98% achieve access to comprehensive management under a model known as heart failure clinics, involving multidisciplinary treatment. This model has proven to be an effective option to decrease the rates of hospitalization, readmission to the emergency room, increased survival and improvement in quality of life. However, the number of heart failure units that currently exist in the country are insufficient in relation to the number of patients suffering from the disease, therefore methodological strategies are applied to determine an alternative solution. Under policies issued for several years by the world health organization, the ministry of health (PAIS) and several leading models of care in the world (for example, the permanent Kaiser model), it is considered that integrating primary care to the second and third level of care is an excellent strategy to improve health outcomes and reduce costs. Therefore, articulating the heart failure units to the first and second level of care is considered a viable option in solving the problem.
