Introduction: Endoscopic retrograde cholangiopancreotography (ERCP) has traditionally been the gold standard for diagnosing and treating biliary diseases. However, this may fail in 5 to 20% of patients, even in referral centers (1). Therefore, cholangioscopy has been introduced over the last few decades as an intervention strategy for difficult bile duct cases which would otherwise require a surgical intervention with a consequently greater risk for the patient due to the inherent complications of this procedure. The objective of this study was to describe the demographic, clinical and paraclinical characteristics and outcomes of individuals who underwent SpyGlass cholangioscopy at the Fundación Cardioinfantil between February 2019 and February 2021. Methods: A retrospective, descriptive study of consecutive patients of all ages who underwent SpyGlass cholangioscopy for various reasons between February 2019 and February 2021. Their demographic and procedural characteristics, outcomes achieved in terms of technical and clinical success, and complications were evaluated. Results: This study found 60 patients who underwent a total of 83 SpyGlass cholangioscopies. Of these, 54 patients (90%) were adults and 10% were children, with a mean age of 51 years [range from 1-85 years]. Most of the patients were females (33; 54.1%). The most common indication was indeterminate stenosis (27.9%), followed by postoperative stenosis (23%) and difficult bile duct stones (21.3%). Technical success was achieved in 100% and clinical success in 90% of the total population. In our cohort, 11.6% of the patients had complications (n=7): six patients had cholangitis and one had hemobilia; all episodes were mild.
