Morbilidad y mortalidad de reintervención por sangrado en cirugía cardiaca, Fundación Cardioinfantil, 2008 - 2017

Background: Excessive bleeding after cardiac surgery continues to be a serious complication, it could cause cardiac tamponade and the need for reintervention. Reintervention for bleeding has been associated to a significant increase in morbidity and mortality. Methodology: Retrospective descriptive cohort study. The population was composed by all patients older than 18 years with reintervention for bleeding after cardiac surgery using cardiopulmonary bypass at Fundación Cardioinfantil from 2008 through 2017. Results: We found 6295 patients in the study institution database; we recruited a sample of 361 patients who had reintervention for bleeding (5.7%). We found a 16% mortality during the whole period (58 patients), compared to 3.81% of patients who did not have reintervention for bleeding. We found that patients who died were older, diabetic, had higher EuroScore values and lower level of hemoglobin. This group of patients had a longer time of cardiopulmonary bypass and received more transfusions. They had more complications such as; sepsis, renal failure, prolonged mechanical ventilation, longer hospital and intensive care unit stays. Conclusion: Reintervention for bleeding is a lethal complication, it represents an important topic which should be controlled with the aim of reducing morbidity and mortality in cardiac surgery, especially in patients with high surgical risk.
