Avaliação da qualidade do serviço ambulatorial especializado oferecido às pessoas vivendo com HIV em município do interior do Rio Grande do Sul

Specialized HIV/AIDS services play a crucial role in reaching the 90-90-90 goal of controlling the HIV epidemic. The objective of this study was to evaluate the quality of outpatient services of two specialized services located in a country town of Rio Grande do Sul state, Brazil, which provide care for people living with HIV (PLHIV). We analyzed the responses to the Qualiaids questionnaire applied in the year in two services. A municipal provision service, identified as specialized unit (UE), and another of federal provision, identified as specialized service (SE). Both services are large, although the SE service has a higher monthly demand. The EU service presented monthly index of missing medical appointments of 13.9% and the SE service of 19%. The follow-up of PLHIV was satisfactory, but both services identified difficulties in the strategy to help PLHIV related to antiretroviral treatment. The EU service presented a greater diversity of professionals involved in the care of PLHIV. Although the SE service team is comprised only by infectologist doctors and pharmacist, this service has the support of multiprofessional residency that expands the professional staff available to the care of PLHIV in the service. The lack of supplies and the difficulty of access to some tests and medical specialties were indicated by both services. The managerial difficulties related to medicines were only signaled by the SE service. The technical coordination in the EU service is carried out by a professional who also acts directly in the assistance. The SE service does not have technical coordination. The characterization and identification of the main potentialities and fragilities of the specialized services that serve PLHIV in the municipality show a satisfactory performance. However, actions that improve the care provided and the level of retention of PLHIV under treatment, in both evaluated services, are necessary.
