This paper presents the development of a systematic process to identify and classify failure modes using the methodology ANALYSIS FAILURE MODES AND EFFECTS (FMEA) as a useful procedure to improve the quality and safety of care services.
254 failure modes were analyzed in the emergency department of a Level II ESE Capital District; we used a selection tool called Matrix prioritization processes used by the Hospital Management Center with adaptations according to institutional needs.
227 of them were scored corresponding to a 89.37%. We interviewed 48 physicians, 27 nurses, 27 nursing assistants and orderlies 9, for a total of 111 employees accounting for 30% of all emergency service personnel. Sheet quality control (acceptance) was generated for the year with a total share of 85%, with the result that 102 people of the 111 respondents fill correctly all fields on the FMEA.
We sought to implement the use of FMEA methodology as a tool for managing and improving business processes, conducting a pilot test selected process and assessing whether this methodology could be applied to other processes of care.
It was noted that all the failure modes of criticality the highest value found in the range of 45 points. 11 failure modes were identified in this category, which are two partners are both associated with patient access to the institution, seven related to User Registration and Entry, one related to the planning of care and two related to implementation of treatment.
