Introduction
Neonatal hyperglycemia is associated with negative outcomes. There is controversy over the optimal management of this clinical entity and this issue has not been addressed through a systematic review.
Objective
To evaluate the effectiveness of interventions aimed at the management of neonatal hyperglycemia in very low birth weight preterm infants through a systematic review of the literature.
Methodology
A systematic review of the literature was carried out. The search for clinical trials was conducted through the Medline (Pubmed platform), Embase, Bireme and Cochrane library databases. We performed a qualitative analysis of the information and an analysis of the sources of bias of the studies.
Results
Six trials were included. Great clinical heterogeneity was found in terms of definitions of hyperglycemia and interventions used. Intensive insulin therapy is associated with increased mortality, adjustments in the administration of amino acids and lipids in parenteral nutrition are associated with a lower proportion of cases of hyperglycemia.
Conclusion
There is no conclusive evidence on the best management of hyperglycemia in the very low birth weight neonate. Intensive insulin therapy is associated with increased mortality, however, more studies are needed to draw a definitive conclusion.
