Tratamiento de hematoma etmoidal progresivo con formalina al 10%, utilizando una pipeta rígida transnasal guiada endoscópicamente: reporte de cuatro casos

Se realizó la presente investigación para evaluar la eficacia de
la administración de formalina al 10% usando una pipeta rígida
intranasal de fabricación artesanal, guiada por un endoscopio,
la misma consistió en una investigación descriptiva. Se describieron
cuatro casos clínicos tratados con formalina al 10%
aplicando cantidades de dos a ochenta mililitros, con intervalos
entre ocho y treinta y cinco días. De los cuatro casos, uno respondió
exitosamente al tratamiento, disminuyéndose casi en
su totalidad la masa del hematoma etmoidal progresivo (HEP)
permitiéndole a la yegua continuar favorablemente la campaña
pistera. El segundo y tercer caso, correspondiente a dos yeguas
madres tratadas, las cuales pudieron finalizar su lactancia;
el tratamiento logró disminuir notablemente el tamaño del
HEP en una de ellas, mientras que en la otra sólo se pudo reducir
un poco el HEP y retardar su crecimiento. La yegua madre
del segundo caso posterior al destete fue retirada del tratamiento,
se le efectuó la eutanasia debido al gran tamaño del
HEP y la poca mejoría, la otra yegua madre luego del destete
también fue retirada del tratamiento. En lo que se refiere al
cuarto caso, al caballo se le hicieron tres intentos para colocarle
el tratamiento a través de la pipeta pero no pudo ser aplicada
la formalina al 10%. En general, en ninguno de los animales
tratados hubo complicaciones post tratamiento. Dentro de
la investigación se pudo observar la presencia de la lesión en
dos animales menores de cinco años y las lesiones se manifestaron
de manera unilateral; tres de los animales tratados
presentaron secreciones nasales purulentas y sanguinolentas
en reposo, dos con deformidad facial y uno de ellos mostró
exoftalmos. En un caso, la lesión ocupaba toda la cavidad de
los senos paranasales y cavidad nasal. Se concluye que se
puede fabricar esta pipeta para tratar animales de gran tamaño
o con hematomas muy grandes, donde la distorsión producida
por el mismo permite el acceso de la pipeta. Se logró administrar

10% formalin using a stiff intranasal pipette of artisanal fabrication
guided through an endoscopy. The experiment was realized
as a descriptive research. Four cases were treated with
10% formalin applying amounts from two to eighty milliliters,
with intervals between eight days and thirty-five days. To the
four cases, one of them responded to treatment successfully,
diminished almost entirely the mass of Progressive Ethmoidal
(PHE). Haematoma allowing favorably to the mare continue its
tracking campaign. The second and the third of cases corresponding
to two mares treated, which could end their lactation,
treatment was able to reduce significantly the size of the PEH
in one, while the other could only slightly reduce PEH and slow
their growth. The mare of the second case was withdrawn of
the treatment after weaning and was performed euthanasia
due to the large size of the PEH and little improvement; the
mare of the third also was withdrawn of the treatment after
weaning. As regards the equine of the fourth case, the treatment
could not be to apply it, although three times attempted to
introduce the pipette intranasal way in the horse. In general,
none of the treated animals had complications after treatment.
During the research two animals were under five years old, in
all of them, observed the presence of the unilaterally lesion.
Three of the treated animals showed bloody purulent nasal secretions
at rest, two with facial deformity and one showed
exophthalmos. In one case, the lesion occupied the entire cavity
of the sinuses and nasal cavity. The conclusion is that pipette
may be made to treat large size animals with very large
hematomas, where the distortion produced by the hematoma
enables access by pipette. Administer treatment was achieved
three of the four animals with PEH.
