Percepção dos responsáveis sobre a importância da caderneta de saúde da criança

A avalição da condição nutricional de uma criança é essencial para o acompanhamento
do seu desenvolvimento. Sendo assim, se faz importante a utilização de instrumentos
padronizados e individualizados por sexo e idade, além da integração entre equipe de
saúde e a família para proporcionar um cuidado integral na infância. Neste contexto, o
Cartão da Criança (CC) foi revisado e substituído pela Caderneta de Saúde da Criança
(CSC) em 2005, ambos elaborados e atualizados com o objetivo de tornarem-se
importantes estratégias de vigilância. Neste contexto o presente estudo teve como
objetivo avaliar a percepção e práticas dos responsáveis sobre a importância da
caderneta de saúde da criança. Participaram da pesquisa 240 responsáveis por crianças
menores de dez anos de idade, no município de Vitória de Santo Antão - PE. Os dados
foram coletados por meio de entrevista com o responsável direto pela criança, durante
as consultas nas Unidades Básicas de Saúde (UBS) ou em visitas domiciliares. A maior
parte dos entrevistados era pai ou mãe da criança (92,5%), a maioria das mães era
multíparas (63,9%) e viviam com renda per capita inferior a ¼ de salário mínimo
(90%). Aproximadamente 57% dos ACS solicitavam a CSC na visita domiciliar e
70,3% realizavam medidas antropométricas. Além disso, 55,4% dos responsáveis não
receberam orientações para procurar a UF quando identificasse alguma alteração de
peso e altura na criança. Dessa forma, apesar da maioria dos responsáveis considerarem
a CSC importante (96,7%), ainda é preocupante os conhecimentos que os mesmos
possuem sobre o instrumento. O vínculo paterno/materno (p<0,038), ter companheiro
(p<0,041), residir com até quatro pessoas no mesmo domicílio (p<0,037) e a não
realização de medida antropométrica no domicílio pelo ACS (p<0,018) apresentaram
relação estatisticamente significante com a prática dos responsáveis de levar a CSC aos
atendimentos. Assim, a falta de valorização do instrumento, ausência de formação
continuada para os profissionais de saúde, além da vulnerabilidade social das famílias,
são determinantes importantes da percepção e práticas relacionadas à CSC. A adequada
utilização da CSC pelos profissionais possibilita maior valorização e apropriação do
instrumento pela família, favorecendo sua maior adesão e co-responsabilização pelas
ações de vigilância The evaluation of the nutritional condition of the children is essential for the follow-up
of their development. Thus, it is important the use of standardized instruments and
individualized by gender and age, and the inclusion of a healthcare team and the family
to provide a holistic care during the childhood. In this context, the Child’s Card (Cartão
da Criança - CC) was revised and replaced for the Child Health Handbook (Caderneta
da Saúde da Criança - CSC) in 2005, both drawn up and updated with the objective to
become important strategies for the monitoring. In this context, this study had the
objective to evaluate the guardians’ perception and practices about the importance of the
child health handbook. In this research were involved 240 guardians of children under
the age of ten, in Vitória de Santo Antão – PE municipality. The data were collected by
interviewing the guardians into direct contact to the children, during the visits to
Healthcare Units (Unidades Básicas de Saúde – UBS) or home visits. Most part of the
interviewers were father or mother of the child (92,5%), the most part of the mothers
were multiparas (63,9%) and were living with a per capita income below ¼ of the
minimum wage (90%). Approximately 57% of the ACS requested the CSC during the
home visits and 70,3% realized anthropometrics measurements. In addition, 55,4% of
the guardians did not receive any orientation to look for a UF when identify any
alteration in the child’s weight and height. In this way, although most of the guardians
considered the CSC as being important (96,7%), still worrying the knowledge that they
have about this instrument. The paternal/maternal link (p<0,038), have a partner
(p<0,041), live with until four people at the same home (p<0,037) and the not realizing
of the anthropometric measurement at home by the ACS (p<0,018) presented a
statistically significant relation to the guardians’ practices of taking the CSC for the
attendances. Thus, the lack of valuation, the absence of continuous training for the
health professionals, besides the social vulnerability of the families, are important
determinants for the perception and practices related to the CSC. The right use of the
CSC by the professionals provides more valorization and appropriation of this
instrument by the family, favoring a higher adhesion and a co-responsibility over the
actions of the monitoring.
