Orientadores: Francisco Haiter Neto, Cínthia Pereira Machado Tabchoury Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo do presente estudo foi comparar o desempenho de imagens por tomografia computadorizada de feixe cônico (TCFC) e radiografia intraoral digital no diagnóstico de lesões de cárie proximais induzidas artificialmente sob as restaurações de amálgama e resina. Trinta e três dentes humanos extraídos, dentre eles 2 pré-molares e 31 molares, foram selecionados. Preparos cavitários classe II foram confeccionados e restaurados com amálgama de prata em 30 dos molares (grupo Amálgama). Desses, quinze molares foram aleatoriamente selecionados e tiveram suas porções coronárias e radiculares revestidas com verniz e apenas uma área circular (7 mm²), na interface restauração/esmalte hígido na face mesial, ficou exposta à solução desmineralizante por 90 dias. Phanton de quatro dentes (dois pré-molares e dois molares) foram confeccionados. Imagens por TCFC foram adquiridas no aparelho k9000, i-CAT next generation com dois tamanhos de voxel (0,4 mm e 0,2 mm). Além disso, radiografias periapicais foram obtidas nos sistemas digitais: Digora Optime, VistaScan e Kodak RVG 6100. Após a aquisição de todas as seis modalidades de imagem, as restaurações de amálgama foram substituídas por resina composta (grupo Resina) e novas imagens foram obtidas. Três avaliadores realizaram a avaliação das imagens em relação ao diagnóstico de lesão de desmineralização sob as restaurações, em grupos de 15 imagens aleatórias. As concordâncias intra e interobservador foram calculadas a partir do Coeficiente de Correlação Intraclasse. Sensibilidade, especificidade, acurácia e área sob a curva ROC foram calculadas. A influência do material restaurador e da modalidade de imagem utilizada foi verificada através de um modelo de análise de variância multifatorial (ANOVA two-way), comparações pareadas foram realizadas por meio do teste de Tukey, considerando-se um nível de significância de 5%. A concordância intraobservador variou de ruim a substancial (0,105 - 0,779) e a concordância interorbservador variou de ruim a regular (0,141 - 0,595). Não houve diferença estatisticamente significante de sensibilidade e especificidade do mesmo material comparado nas diferentes modalidades de imagem (p = 0,301 e 0,784, respectivamente) ou quando os dois materiais foram comparados em uma mesma modalidade (p = 0,306 e 0,437, respectivamente). Já com relação à acurácia e áreas sob as curvas ROC, foi encontrado diferença estatisticamente significante quando os dois materiais foram comparados nas modalidades iCat0.4/iCat0.2 e VistaScan/iCat0.4, respectivamente. Além disso, houve diferença estatisticamente significante quando os valores do grupo Amálgama foram comparados entre as diferentes modalidades de imagem. Concluiu-se que as imagens por TCFC apresentaram performance compatível com as técnicas radiográficas intraorais para detecção de desmineralização sob restaurações. Contudo, o protocolo de aquisição das imagens, como tamanho do FOV e do voxel, e o material restaurador utilizado pareceu influenciar na performance Abstract: The aim of this study was to compare the performance of cone beam computed tomography (CBCT), digital intraoral radiography in the diagnosis of artificially induced proximal caries lesions under amalgam and composite restorations. Thirty-three extracted human teeth, including two premolars and 31 molars, were selected. Class II cavity preparations were made and restored with amalgam in 30 molars (Amalgam group). Of those, fifteen molars were randomly selected and coated with nail polish such that only a circular area (7 mm²) of the restoration/enamel interface, at the mesial surface, was exposed to a demineralizing solution for 90 days. Phanton with four teeth (two premolars and two molars) were created. CBCT images were acquired using a k9000 (0,076mm voxel size) and an i-CAT next generation (0.4 mm and 0.2 mm voxel sizes) scans. In addition, periapical radiographs were obtained in three digital systems (Digora Optime, VistaScan and Kodak RVG 6100). Following the acquisition of all six imaging modalities, the amalgam fillings were replaced by composite (Composite group) and new images were obtained. Three observers interpreted the images by searching for demineralized areas under the restorations, in groups of 15 random images of the same technique, regardless of the restorative material and the condition under the restoration. Intra and interobserver agreements were calculated by intraclass correlation coefficient. Sensitivity, specificity, accuracy and area under the ROC curve (AUC) were also calculated. The influence of the restorative material and the type of image used was verified through a multifactorial analysis of variance model (ANOVA two-way), and paired comparisons were performed using Tukey test, considering a significance level of 5%. The intraobserver agreement ranged from poor to substantial (0.105 to 0.779) and the interorbservador agreement ranged from poor to regular (0.141 to 0.595). There was no statistically significant difference in sensitivity and specificity for the same material compared in different imaging modalities (p = 0.301 and 0.784, respectively) or when the materials were compared in the same modality (p = 0.306 and 0437, respectively), except for film¿s sensitivity (p = 0.013). On the other hand, statistically significant differences were found for accuracy and AUC when the materials were compared in the categories iCat0.4/iCat0.2 and VistaScan/iCat0.4 respectively. In addition, there was statistically significant difference in the Amalgam group when the accuracy and AUC were compared between the imaging modalities. In conclusion, CBCT images showed consistent performance, comparable with intraoral radiographic techniques, to detect demineralization under restorations.
