Orientador: Eliete Maria Silva Dissertação (mestrado profissional) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Passados quase 20 anos dos primeiros relatos de sua implantação, estudos apontam que coexistem inúmeras formas de conceber, organizar e fazer o Acolhimento. Embora haja relatos de rearranjos positivos do trabalho e avanços no sentido da ampliação do acesso com a implantação do Acolhimento, predomina a lógica hegemônica centrada no saber médico, com baixa autonomia e resolubilidade sugerindo que esse permanece distante de assumir o papel transformador do modelo de atenção. Este estudo busca, por meio de uma pesquisa-ação, descrever e intervir sobre as práticas de acolher em um Centro de Saúde de Campinas com o intuito de evidenciar entraves e perspectivas do Acolhimento e refletir sobre a mudança nas práticas de saúde. Destaca-se predomínio de questões relacionadas à relação usuários-trabalhadores. Reconhece-se o Acolhimento como sendo postura ética e acolhedora mas admite-se a existência de uma sala com esse nome onde, na prática, acontecem a triagem dos casos que passarão por avaliação médica, por vezes funcionando como uma barreira ao acesso à consulta médica. Apontam-se dilemas como o medo de "abrir as portas" e "virar um Pronto-Socorro" como elementos presentes no discurso de trabalhadores. O contexto de crise do capitalismo marcado por corte nos gastos públicos, refletem nas condições do trabalho e ocasionam restrições no acesso impactando diretamente usuários, gestores e trabalhadores. A Enfermagem vem se consolidando como "linha de frente" do Acolhimento, atendendo ilimitadamente a todos que chegam e o trabalho médico permanece aprisionado à lógica agenda/consulta, sendo fortemente centrado no limite de vagas. Hostilidades, agressões físicas e verbais se fazem presente demarcando relações entre usuários e trabalhadores, por vezes, reciprocamente violentas. Deliberou-se por agir em duas frentes: uma relacionada à forma inicial de receber o usuário e outra ligada à elaboração de protocolos. Nenhuma proposta apresentou caráter de rompimento com a lógica hegemônica atual centrada no trabalho médico, com consulta e procedimentos. Observou-se facilidade em identificar problemas e propor soluções, mas dificuldades na implementação. Por fim, duas décadas depois, notam-se poucas mudanças em relação aos achados iniciais de investigação sobre o Acolhimento, motivo pelo qual acredita-se ter chegado o momento de reacender suas energias adormecidas ou dar um passo à frente rumo à construção de novas diretrizes que superem os obstáculos apontados para seguir caminhando, mesmo que em cenário adverso, rumo a serviços de saúde verdadeiramente acolhedores. Palavras-chave: Acolhimento, Atenção primária à saúde, prática profissional Abstract: After nearly 20 years of the first reports of its implementation, studies indicate that co-exist many ways to conceive, organize and make user embracement, what we call in Brazil "Acolhimento". Although there are positive rearrangements reports of work and progress towards the expansion of access since the implantation of "Acolhimento". Still is predominant an hegemonic logic centered in the medical knowledge, with low autonomy and resolution of other professionals suggesting that remains far from taking the transformative role of heath care model. This study aims, through an action research, describe and intervene in the practices of "Acolhimento" in a primary health care center of Campinas City seeking to highlight obstacles and perspectives of the reception and think about the change in health practices. Emphasis is given to issues related to the user-worker relationship. The "Acolhimento" is acknowledged as ethical and welcoming professional attitud but admits the existence of a room with this name where, in practice, the screening of cases that will undergo medical evaluation, sometimes acting as a barrier to access to medical consultation. Dilemmas such as "opened doors" and fear of "becoming a Emergency Service" are elements present in the workers discourse. The crisis context of capitalism marked by cutbacks in public spending reflects on the conditions of work and causes restrictions on access that directly impact the daily life of the service. Nursing has been consolidating itself as the "front line" of the "Acolhimento" attending unlimitedly to all who arrive and the medical work remains imprisoned to the agenda / consultation logic being strongly focused on the limit of vacancies. Hostilities, physical and verbal aggressions are present, marking a sometimes abusive user-worker relationship. It was decided to act on two perspectives: one related to the initial way of receiving the user and another linked to the elaboration of protocols. It was easy to identify problems and propose solutions but difficulty to do its implementation. Finally, there are few changes in relation to the initial findings of research on the Reception, which is why it is believed that the moment has come to rekindle their dormant energies or to take a step forward towards the construction of new guidelines that overcome the obstacles aiming to continue walk, even in an adverse scenario, going towards truly welcoming health services Mestrado Política, Gestão e Planejamento Mestra em Saúde Coletiva
