Orientadores: Paulo César Giraldo, Cássia Raquel Teatin Juliato Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Introdução: A recomendação para o treinamento da musculatura do assoalho pélvico (TMAP) tem sido preconizada pela fisioterapia uroginecológica para auxiliar no tratamento e prevenção de diversas doenças. Entretanto, ainda é desconhecida sua ação sobre o ecossistema vaginal. A principal queixa relacionada a alteração do ecossistema vaginal é o corrimento, frequentemente altera a vida sexual e orgânica das mulheres. O ácido lático -L (AcL -L), é uma substância presente em abundância na cavidade vaginal e exerce diversas funções imunológicas da flora vaginal. Ele encontra-se associado nas vulvovaginites (VV), como na Candidíase Vaginal (CVV), na Vaginose Bacteriana (VB), na Vaginose Citolítica (CV), porém, ainda não sabe como o TMAP pode influenciar as concentrações do AcL -L. Portanto, a avaliação do ecossistema vaginal sob a ação do TMAP e a função sexual de mulheres com VV, principalmente nas com história de recorrências, e suas possíveis associações com disfunções da musculatura do assoalho pélvico (MAP) poderiam trilhar novos caminhos para a fisioterapia do assoalho pélvico. Desta forma, proporcionando uma MAP mais tonificada e resistente, o que contribuiria para uma flora vaginal saudável, e também melhoraria a sexualidade dessas mulheres. Objetivo: Avaliar a influência do TMAP no ecossistema vaginal e verificar a função sexual de mulheres com e sem VV. Materiais e Métodos: Ensaio clínico que envolveu 120 mulheres atendidas no Ambulatório de Infecções Genitais e no ambulatório de Planejamento Familiar do Departamento de Tocoginecologia da Faculdade de Ciências Médicas da Universidade Estadual de Campinas (DTG/FCM/Unicamp) atendidas no período de agosto 2014 à julho de 2015. Foram incluídas mulheres no menacme, entre 18 e 45 anos, com e sem história de infecções genitais, que já haviam iniciado sua vida sexual. Após admissão no estudo, coletou-se swab vaginal para análise microbiológica (bacterioscopia vaginal e cultura para fungos) e avaliou-se a MAP através do teste manual - PERFECT. Todas as mulheres receberam a intervenção do TMAP (sequência de contrações perineais ¿ duração de 45 minutos). Antes e depois do TMAP foram analisados: pH vaginal, temperatura vaginal e coletou-se mais um swab da parede vaginal lateral, que foi diluído em 1 ml de PBS (phosphate buffer saline), processado e congelado para posterior mensuração do AcL -L. Foi realizado um estudo de Corte Transversal para avaliar a função sexual dessas mulheres através do questionário FSFI - Female Sexual Function Index. Dos casos coletados, após análise do diagnóstico laboratorial e aplicação dos critérios de exclusão foram selecionadas 80 mulheres com: CVV (n=22), VB (n=18), e sem infecção genital (Controle n=40) para analisar o efeito do TMAP sobre ecossistema vaginal. Para analise da influência da história de corrimento vaginal recorrente sobre a função sexual e a associação com a MAP. Na análise estatística foram utilizados os seguintes testes: Teste exato de Fisher, Qui Quadrado, Anova para medidas repetidas, teste de Tukey, T pareado, Kruskal Wallis, Mann Whitney. Para verificar as variáveis que afetam a função sexual (FSFI) foram utilizados os testes de regressão linear simples e múltipla e regressão logística múltipla. O estudo foi aprovado pela Comissão de Ética e Pesquisa da Unicamp Resultados: Imediatamente após o TMAP, as concentrações de AcL -L e o pH aumentaram nas 80 mulheres (AcL -L p=0,0001 e pH = 0,0006), no grupo controle (p?0,0001 AcL -L e pH) e com CVV (AcL -L p?0,0001 e pH p=0,0062). O TMAP não influenciou significativamente esses parâmetros na VB e a temperatura vaginal não alterou significativamente em nenhum dos grupos após o exercício (p?0,05). Ter história de corrimento vaginal recorrente mostrou ser um fator preditivo para disfunção sexual (DS) (p?0,05). Os 2 grupos com história de corrimento sem e com infecção genital atual apresentaram todos os domínios afetados (p?0,05) e o score total abaixo de 26,55 indicativo de DS. Não houve correlação dos valores da força e resistência da MAP com ter DS. O grupo com infecção genital atual (VB+CVV+VC) apresentou menor número de contrações mantidas da MAP (p=0,027) quando comparadas ao grupo sem infecção atual (com e sem história de recorrência de corrimento vaginal). Conclusão: O TMAP aumenta a concentração de AcL -L e pH vaginal em mulheres sem infecção vulvovaginal e com CVV. Mulheres com história de corrimento vaginal recorrente apresentam DS. A força e a resistência da MAP não apresentaram correlação com a presença de DS. A MAP de mulheres com infecção genital aguda fadiga precocemente Abstract: Introduction: pelvic floor muscle training (PFMT) has been recommended by urogynecological physical therapy to assist in the treatment and prevention of several disorders, such as urinary incontinence, vulvar pain and sexual dysfunction. However, the action of PFMT on the vaginal ecosystem is still unknown. The main complaint related to alteration in the vaginal ecosystem is vaginal discharge, frequently disturbing sexual and organic lives of these women. Lactic acid-L (L-Lac) is widely present in the vaginal cavity. The substance exerts diverse immunologic functions of the vaginal flora. It has been associated with vulvovaginitis (VV), vulvovaginal candidiasis (VVC), and bacterial vaginosis (BV), cytolytic vaginosis (CV). However, the influence of PFMT on L-Lac concentrations is largely unknown. Therefore, evaluation of the vaginal ecosystem under the action of PFMT and sexual function in VV patients, particularly those with a history of recurrences, and potential associations with pelvic floor muscle (PFM) dysfunction could pave the way for pelvic floor physical therapy. Thus, a more resistant and firmer PFM tone could contribute to a healthy vaginal flora, and also improve sexuality of these women. Objective: To assess the influence of PFMT on the vaginal ecosystem and examine sexual functioning in women with and without VV. Materials and Methods: A clinical trial was conducted including 120 sexually active women seen at the Genital Infection Outpatient Facility of the Family Planning Division in the Department of Obstetrics and Gynecology at the State University of Campinas School of Medicine (DTG/FCM/Unicamp) seen from August 2014 to July 2015. Included in the study were women in the menacme, aged 18 to 45 years, with and without a history of genital infections. After study admission, a vaginal swab was collected for microbiological analysis (vaginal bacterioscopy and fungal culture) and PFM was evaluated through the manual test - PERFECT. All women received PFMT intervention (a sequence of perineal contractions lasting 45 minutes). Before and after PFMT, the following parameters were analyzed: vaginal pH and vaginal temperature. An additional swab was collected from the lateral vaginal wall, that was diluted in 1 ml of PBS (phosphate buffer saline), processed and frozen for subsequent L-Lac measurement. A cross-sectional study was conducted to assess sexual functioning of these women through the FSFI (Female Sexual Function Index) questionnaire. Of the sampled cases, after laboratory diagnosis and application of exclusion criteria, 80 women with VVC (n=22), BV (n=18), and those without genital infection (control n=40) were selected to analyze the effect of PFMT on the vaginal ecosystem, as well as the influence of history of recurrent vaginal discharge on sexual function and its association with PFM. For statistical analysis, Fisher¿s exact test, chi-square test, repeated measures Anova, Tukey¿s test, paired t-test, Kruskal-Wallis and the Mann-Whitney test were used. To analyze variables that affect sexual function (FSFI), simple linear regression and multiple logistic regression tests were used. The study was approved by the Research Ethics Committee of the Institution (Unicamp) Results: Immediately after PFMT, L-Lac concentrations and pH levels increased in the 80 women (L-Lac p=0.0001 and pH = 0.0006), in the control group (p?0.0001 L-Lac and pH) and in VVC patients (L-Lac p?0.0001 and pH p=0.0062). PFMT did not significantly influence these parameters in BV patients. Vaginal temperature was not significantly altered in any of the groups after exercise (p?0.05). A history of recurrent vaginal discharge was shown to be a predictive factor for sexual dysfunction (SD) (p?0.05). All domains were impaired (p?0.05) and a total score below 26.55 indicative of SD were observed in the 2 groups that had a history of recurrent vaginal discharge with and without current genital infection. There was no correlation between PFM strength/contraction values and having SD. The group with current genital infection (BV+VVC+CV) had a lower number of sustained PFM contractions (p=0.027) than the the group without current infection (with and without a history of recurrent vaginal discharge). Conclusion: PFMT increases L-Lac concentration and vaginal pH in women without vulvovaginal infection and in VVC patients. Women with a history of recurrent vaginal discharge had SD. PFM strength and resistance did not correlate with the presence of SD.
