Orientador: Andréa Trevas Maciel-Guerra Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: O diagnóstico diferencial entre as disgenesias gonadais parcial 46,XY (DGP) e mista (DGM) foi estabelecido inicialmente em termos histológicos, porém há diferenças clínicas e prognósticas em presença de mosaicismo com linhagem 45,X. Apesar disso, não há uniformidade na literatura médica quanto à classificação e diferenciação entre as duas condições. O objetivo deste trabalho foi analisar o quadro clínico e laboratorial de pacientes 46,XY com ambiguidade genital decorrente de disgenesia testicular com e sem linhagem 45,X e comparar essas condições em termos clínicos e de prognóstico. Para isso, foram levantados todos os casos de disgenesia testicular atendidos em nosso serviço de 1989 a 2013. Casos de DGP com suspeita de mosaicismo tiveram a análise citogenética aprofundada, incluindo a técnica de FISH; esta também foi empregada em pacientes com DGP ainda acompanhados no serviço. Os pacientes foram separados em dois grupos (com e sem linhagem 45,X), comparados em relação a histologia gonadal, anatomia genital interna e externa, função hormonal das gônadas, presença de dismorfismos e doenças associadas, crescimento, puberdade e prognóstico de fertilidade. A casuística final consistiu de 61 pacientes, 25 com DGM e 36 com DGP. Os grupos não diferiram em relação à idade da primeira consulta, história gestacional e familiar, grau de virilização dos genitais externos, posição, histologia e função hormonal das gônadas, evolução puberal espontânea e necessidade de reposição hormonal, presença de comorbidades e prognóstico de fertilidade. Houve diferença significativa quanto ao sexo final (mais frequentemente feminino nas DGM), presença de útero (mais frequente nas DGM), idade materna mais avançada (nas DGP), menor peso e comprimento ao nascer (nas DGM) e presença de baixa estatura (mais frequente nas DGM). Portanto, DGP e DGM foram indistinguíveis em termos de histologia e função gonadal e características genitais, exceto a maior frequência de útero na DGM, que sugere disfunção mais grave das células de Sertoli. Diferiram, porém, em relação ao crescimento pré e pós-natal, em relação ao qual pacientes com DGM requerem medidas terapêuticas específicas. Assim sendo, a antiga classificação baseada na histologia deve ser abandonada em favor daquela baseada no cariótipo, e a busca da linhagem 45,X deve ser aprofundada em todos os pacientes com disgenesia testicular 46,XY Abstract: Differential diagnosis between XY partial (PGD) and mixed gonadal dysgenesis (MGD) was initially established by histological evaluation; however, when there is a 45,X lineage there are differences not only in clinical aspects but also in prognosis. Nonetheless, many present-day studies still refer to this old histological classification. The aim of this work was to analyze clinical and laboratorial picture of patients with genital ambiguity due to testicular dysgenesis, with and without a 45,X lineage, and compare these conditions in terms of phenotype and prognosis. We selected all patients with a diagnosis of testicular dysgenesis who were seen in our service between 1989 and 2013. Those with previous diagnosis of PGD who had a clinical suspicion of hidden mosaicism were subject to deeper cytogenetic analysis, including FISH; this technique was also employed to screen PGD patients who were still being followed at our service. The patients were then divided in two groups (with and without a 45,X cell line), which were compared in regard to gonadal histology, anatomy of external and internal genitalia, gonadal hormone function, presence of dysmorphic features and associated conditions; growth, puberty and fertility prognosis. Our sample included 61 patients, 25 with mosaicism (MGD) and 36 with an homogenous 46,XY karyotype (PGD). There were no differences between the groups in terms of age at the first visit, gestational and family history, degree of external virilization, position and histology of gonads, gonadal hormone function, spontaneous pubertal development and need for hormonal replacement, presence of associated conditions and fertility prognosis. There were significant difference regarding sex of rearing (more often female in MGD); presence of uterus (more common in MGD); higher maternal age (in PGD); lower birth weight and length (in MGD) and short stature (more frequent in MGD). Thus, PGD and MGD were indistinguishable in terms of gonadal histology and function and genital features, except for the higher frequency of uterus in MGD, which may indicate more severe dysfunction of Sertoli cells. They did differ in terms of pre and post-natal growth; in this regard, patients with MGD require specific therapeutic measures. Therefore, the old classification based on histological findings should be abandoned in favor of that based on chromosome constitution, and screening for a 45,X lineage should be thorough in all patients with 46,XY testicular dysgenesis Doutorado Saude da Criança e do Adolescente Doutora em Ciências
