Orientador: Carlos Emilio Levy Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: A fibrose cística (FC) é uma doença genética autossômica recessiva, envolvendo a produção de muco mais espesso e viscoso, devido a um defeito no transporte de água, cloro e sódio nas células epiteliais. Essa alteração resulta em inúmeras manifestações e complicações, destacando-se a doença pulmonar progressiva, que desempenha papel principal na morbidade e mortalidade dos pacientes. O aumento da expectativa de vida desses pacientes permitiu o reconhecimento de uma variedade de outros patógenos, como Mycobacterium "não tuberculosis" (MNT) e alguns fungos filamentosos, destacando o Aspergillus fumigatus, cuja principal manifestação nessa população é a aspergilose broncopulmonar alérgica (ABPA). Assim, é crescente o interesse pelo o impacto do A. fumigatus em manifestações respiratórias, bem como do relato de recuperação de MNT no trato respiratório de pacientes com FC. O presente trabalho levantou dados atuais de prevalência de micobactérias, fungos oportunistas e ABPA e propôs uma sistematização para investigação laboratorial, acompanhamento e critérios para tratamento de pacientes com risco de infecções por estes agentes, além de identificar as principais espécies isoladas e sua importância clínica nos pacientes atendidos nos ambulatórios de fibrose cística do HC Unicamp. Foram incluídos 117 pacientes que forneceram escarro para análise em cultura microbiológica para micobactérias e fungos, durante o período de março de 2014 a dezembro de 2015. A investigação de ABPA iniciou-se a partir de dosagens de IgE total. Foram encaminhados para exames de sorologia específica, os pacientes que apresentaram IgE ? 500 UI/ml e/ou apresentaram quadros de exacerbações pulmonares relacionados à presença do Aspergillus sp. ou sem resposta a antibioticoterapia. A partir do levantamento dos dados na literatura a respeito da investigação de micobactérias e ABPA na população com FC, foi elaborado um protocolo de busca sistemática desses agentes adaptado ao nosso serviço de referência. Do total de pacientes incluídos no estudo, sete apresentaram pelo menos uma cultura positiva para micobactérias e 80 apresentaram pelo menos uma cultura para fungo oportunista. Das espécies de micobactérias mais isoladas, tivemos complexo M. abscessus e M. avium, enquanto que Aspergillus fumigatus e Candida albicans foram as espécies fúngicas com maior isolamento. Oitenta dos 117 pacientes fizeram dosagem de IgE total e 17 realizaram investigação completa para ABPA, entretanto nenhum caso preencheu os critérios diagnósticos propostos por Stevens et al. (2003). Diante da ausência de casos de ABPA, procuramos entender outros possíveis significados clínicos do Aspergillus sp. em nossa população. Possivelmente, quatro casos configurem sensibilização ao Aspergillus sp. e dois podem ser classificados como bronquite por Aspergillus sp. Concluímos que a prevalência de MNT foi de 6%, de Aspergillus sp. foi de 53% e 29% de Candida sp. No período estudado, não foi possível caracterizar nenhum caso de ABPA entre os pacientes investigados. Outros quadros de aspergilose pulmonar, como sensibilização e bronquite por Aspergillus sp., pareceram ser mais frequentes e merecem melhor investigação em nossa população. A implantação de um protocolo de busca sistemática da infecção por MNT e ABPA permitiu conhecer a prevalência e importância desses agentes na FC em nosso meio, bem como sistematizar seu diagnóstico e condutas Abstract: The cystic fibrosis (CF) is genetic disease that involves thick and sticky mucus buildup due to a water, chlorine and sodium conduction malformation in the epithelial cells. This malformation results into several manifestations and complications, hilighting the progressive pulmonary disorders responsible for most morbidity and mortality of patients. The increase of life expectancy for these patients allowed the recognition of many other pathogens such as nontuberculous mycobacteria (NTM) and some filamentous fungi, emphasizing Aspergillus fumigatus which main manifestation in these population is Allergic Bronchopulmonary Aspergillosis (ABPA). As such, the interest in A. fumigatus impact over respiratory manifestations has been increasing as well as recovery reports from CF patients. In the present study, current data about mycobacteria, opportunistic fungi and ABPA prevalence were gathered and was proposed a systematization for the laboratory investigation, follow-up and criteria for the treatment of CF patients at risk of getting infections by these agents, and also identifying the main isolated species and its clinical importance in patients who were attended at CF outpatient clinic at Unicamp. It includes 117 patients who provided sputum for the analysis of mycobacteria and fungi microbiological culture from March 2014 to December 2015. The ABPA investigation got started from total IgE levels. The patients who presents either IgE ? 500 UI/m or clinical manifestations as pulmonary exacerbations related to the presence of Aspergillus sp. or no response to antibiotic treatment were forwarded to specific serological tests. From the review of the literature about mycobacteria investigation and ABPA in the CF population, a protocol of systematics research about these agents was designed and applied to our referral service. From the total of studied patients, seven presented at least one culture for mycobacteria e and 80 presented at least one culture for opportunistic fungi. M. abscessus and M. avium complex were the most isolated mycobacteria species and Aspergillus fumigatus and Candida albicans were the most isolated fungi species. Of the 117 patients, 80 had IgE levels and 17 with clinical data suggesting the possibility of ABPA made a complete investigation of ABPA. However none of these cases meet the diagnostic criteria proposed by Stevens et al. (2003). As the result of absence of ABPA cases, we persue to understand other possible clinical meanings about Aspergillus sp. into our population. Possibly, four cases set as Aspergillus sensitivity and two can be classified as Aspergillus bronchitis. So we conclude that the prevalence was 6% of NTM, 53% of Aspergillus sp. and 29% of Candida sp. In the studied period, there was no possibilty to characterize a case of ABPA among the patients. Other manifestations of pulmonary aspergilosis, such as sensitivity and Aspergillus bronchitis seem to be more frequent and deserve a better investigation of our populations. The implementation of the systematic research protocol for infection by NTM and ABPA enabled to meet the prevalence and importance about these agents in the CF in our medium as well as to systematize its diagnostic and conducts.
