Orientador: Christiane Marques do Couto Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Introdução: A perda auditiva pode prejudicar as habilidades auditivas e o desempenho social, cognitivo e emocional do sujeito. Pode ser bilateral, afetando as duas orelhas ou unilateral, afetando apenas uma. Os efeitos da perda auditiva unilateral no reconhecimento de fala em presença de ruído são considerados tão importantes quanto os ocasionados pela perda bilateral. Objetivo: Analisar o reconhecimento de fala em sujeitos adultos com perda auditiva neurossensorial unilateral. Método: A pesquisa foi experimental e de corte transversal. Os sujeitos da pesquisa foram pacientes do Ambulatório de Otorrinolaringologia da UNICAMP. Constituíram a amostra 24 sujeitos com perda auditiva neurossensorial unilateral: oito do sexo masculino, com faixa etária entre 37 a 58 anos de idade e 16 do sexo feminino com faixa etária entre 21 a 57 anos de idade. Como instrumento de avaliação do reconhecimento da fala no silêncio e no ruído foi utilizado o teste HINT Brasil. O HINT é um teste adaptativo, no qual a intensidade do estímulo de fala é diminuída ou aumentada conforme acertos ou erros do sujeito. A intensidade do ruído e a intensidade inicial do sinal de fala é de 65dB(A). Durante a apresentação das quatro primeiras sentenças a intensidade da fala varia 4dB(A). A intensidade da quinta sentença é resultado da média aritmética entre a intensidade da terceira e da quarta sentenças e, a partir da quinta sentença até a vigésima, a intensidade entre as sentenças varia 2dB(A). Foram aplicadas, de modo aleatório, as listas de sentenças nas situações: silêncio, fala com ruído frontal; fala com ruído à direita e fala com ruído à esquerda. Por meio da média ponderada das condições com ruído, foi obtida uma medida global única do reconhecimento da fala no ruído denominada ruído composto. Foi realizada uma análise exploratória dos dados das variáveis por meio de medidas resumo (média, desvio padrão, mínimo, mediana e máximo) e visto se havia relação destas varáveis com o lado da perda. Os grupos foram comparados com uso do teste de Mann¿Whitney. O nível de significância adotado foi de 5%. Não havendo diferença significativa entre as variáveis em relação ao sexo foram constituídos dois grupos mistos: Grupo D com nove sujeitos com perda neurossensorial na orelha direita e Grupo E com 15 sujeitos com perda neurossensorial na orelha esquerda. Resultados: Os resultados evidenciam não haver diferença significativa entre as variáveis de silêncio e de ruído nos grupos do sexo feminino e masculino. Houve diferença significante entre os grupos GD e GE em relação às variáveis: silêncio, ruído à direita e ruído à esquerda, sendo a relação sinal-ruído mais negativa quando o ruído foi proveniente do lado da perda. Conclusão: A comparação entre as condições avaliadas evidenciou que o melhor desempenho no reconhecimento de fala no ruído é quando o ruído localiza-se do lado da orelha comprometida Abstract: Introduction: The hearing loss can harm the listening skills and the social, cognitive and emotional performance of the person. It may be bilateral, affecting both ears or unilateral, affecting only one. The effects of the unilateral hearing loss in speech recognition in the presence of noise are considered as important as the ones caused by bilateral hearing loss. Objective: To analyze the speech recognition in adults with unilateral sensorineural hearing loss. Method: The research was experimental and cross-sectional. People involved in the research were Otolaryngology ambulatory patients from UNICAMP. The sample was constituted in 24 people with unilateral sensorineural hearing loss: 8 males, with age range between 37 to 58 years old and 16 females, with age range between 21 to 57 years old. As a evaluation tool of speech recognition in silence and in noise was utilized the Brasil HINT test. HINT is an adaptive test, in which the speech stimulation intensity is decreased or increased according to the person's accuracies or errors. The noise' s intensity and the initial intensity of the speech signal is 65dB (A). During the four first sentences the speech intensity ranges 4dB(A). The intensity of the fifth sentence is the result of the arithmetic mean between the intensity of the third and fourth sentences and from the fifth sentence on until the twentieth, the intensity between the sentences ranges 2dB(A). The list of the sentences in this situations: silence, speech with front noise, speech with right noise and speech with noise left were applied in random order. By average conditions with noise, a single global measure of speech recognition in noise, called compound noise was obtained. An exploratory analysis of the summary measures through variable data (mean, standard deviation, minimum, median and maximum) was performed and verified if there was connection of this variables with the loss side. Groups were compared with the usage of the Mann-Whitney test. The significance level used was 5%. With no significant difference between the variables according to sex, two mixed groups were recorded: Group D with 9 patients with sensorineural loss in the right ear and Group E with 15 patients with sensorineural loss in the left ear. Results: The result showed no significant difference between silence and noise variables among the female and male groups. There was a significant difference between the GD and GE groups regarding variables: silence, right noise and left noise, with the most negative signal noise ratio when the noise was coming from the side of the loss.
