Orientador: Alexandre Leite Rodrigues de Oliveira Tese (doutorado) - Universidade Estadual de Campinas, Instituto de Biologia Resumo: Avulsão de raízes ventrais (VRA) é uma lesão proximal de motoneurônios medulares na interface do sistema nervoso central (SNC) e periférico (SNP). Tal lesão desencadeia uma cascata de eventos celulares e moleculares que leva à degeneração retrógrada de cerca de 80% dos motoneurônios axotomizados, duas semanas após a lesão. Assim, a VRA tem se mostrado um modelo experimental reprodutível de degeneração de neurônios motores em modelos animais. Diversos trabalhos têm mostrado os benefícios da terapia celular, utilizando células tronco, na neuroproteção sobre os motoneurônios lesados e, consequentemente, no aumento da sobrevivência neuronal. Outros estudos têm mostrado que a terapia com o fator de crescimento de fibroblastos -2 (FGF2) estimula a neurogênese e a plasticidade sináptica em modelos de doenças neurodegenerativas. O presente estudo teve por objetivo conjugar o uso de células tronco com a entrega local do FGF2, avaliando seu efeito neuroprotetor sobre motoneurônios medulares em ratos Lewis após avulsão de raízes motoras. Além disso, investigou-se a eficácia do uso de selante de fibrina como arcabouço capaz de reter as células enxertadas no sítio da lesão. Para isso foram produzidas, por bioengenharia, linhagens derivadas de células tronco embrionárias humanas que superexpressam FGF2 humano de modo induzido, através da doxiciclina, contendo a proteína fluorescente verde como marcador (hES-FGF2-GFP). Os animais foram divididos nos seguintes grupos experimentais: Grupo 1- avulsão de raízes motoras + selante de fibrina; Grupo 2- avulsão de raízes motoras + selante de fibrina + injeção de hES-FGF2-GFP + doxiciclina; Grupo 3- avulsão de raízes motoras + selante de fibrina + doxiciclina; Grupo 4- avulsão de raízes motoras + selante de fibrina + injeção de hES-FGF2-GFP; Grupo 5- avulsão de raízes motoras + selante de fibrina + FGF2; Grupo 6- Avulsão; Grupo 7 ¿ Sem lesão. Após sobrevida de 14 dias foram analisados (1) a expressão de sinaptofisina (marcador de sinapses), de GFAP (marcador de astrócitos) e Iba-1 (reação microglial), através da técnica de imunohistoquímica; (2) a produção dos fatores neurotróficos BDNF e GDNF pelas hESC; (3) sobrevivência de neurônios motores medulares, através da técnica de coloração de Nissl e posterior análise quantitativa (esteriológica) ao microscópio de luz; (4) através do uso de qRT-PCR, a expressão de marcadores da resposta inflamatória, incluindo TNF-? e interleucinas IL1?, IL-6 IL-10, bem como os fatores BDNF e GDNF. Os resultados mostraram que o tratamento com hESC induzidas para superexpressar FGF2 aumentou a sobrevivência neuronal em 23%. A avaliação imunoistoquímica com sinaptofisina mostrou que nos grupos II e V, tratados com as hESC + doxiciclina e FGF2 respectivamente, houve redução significativa da perda de terminais em aposição ao corpo celular, refletindo uma possível diminuição do processo de eliminação sináptica. Além disso, houve também diminuição significativa da marcação anti-GFAP (astrogliose reativa) no grupo tratado com hESC + doxiciclina, em relação aos outros grupos. Um importante achado foi que as hESC-GFP superexpressando FGF2, produziram BDNF no sítio de injeção das células e co-expressaram GDNF na região perimedular. No entanto, a terapia celular com as hESC e FGF2 não produziu diferenças significativas em relação à reação microglial. O grupo tratado com FGF2 apresentou maior expressão da ?2µ e o grupo tratado com hESC + doxiciclina apresentou menor expressão de TNF-? e maior expressão de IL-10, indicativo de microambiente anti-inflamatório. Em conjunto, os presentes resultados apontaram para um efeito neuroprotetor das células tronco embrionárias humanas modificadas para superexpressar FGF2, além de preservação sináptica e diminuição da astrogliose, podendo esses resultados levar a promissores estudos regenerativos, visando melhorar a recuperação motora após lesões do plexo braquial e lombosacral Abstract: Ventral root avulsion (VRA) is a proximal lesion of spinal motoneurons in the central and peripheral nervous system interface. Such injury triggers a cascade of cellular and molecular events leading to retrograde degeneration about of 80% of the axotomized motoneurons two weeks after the injury. Thus, VRA has been a reproducible experimental model of motoneuron degeneration in animal models. Several studies have shown the benefits of cell therapy using stem cells in neuroprotection on the damaged motoneurons and, consequently, the increase of neuronal survival. Other studies have shown that treatment with basic fibroblast growth factors (FGF2) stimulates neurogenesis and synaptic plasticity in models of neurodegenerative diseases. The present study aimed to combine the use of stem cells with the local delivery of FGF2, evaluating their neuroprotective effect on spinal motoneurons in Lewis rats after ventral root avulsion. In addition, we investigated the efficacy of fibrin sealant as a scaffold capable of retaining the grafted cells at the injury site. Thus, was produced, by bioengineering, lineage derived from human embryonic stem cells that overexpress human FGF2-induced manner by doxycycline, containing green fluorescent protein as a marker (hES-FGF2-GFP). The animals were divided into the following groups: Group 1 - ventral root avulsion + fibrin sealant; Group 2 - ventral root avulsion + fibrin sealant + injection of hES-FGF2-GFP + doxycycline; Group 3 - ventral root avulsion + fibrin sealant + doxycycline Group 4 - ventral root avulsion + fibrin sealant + injection of hES-FGF2-GFP; Group 5 - ventral root avulsion + fibrin sealant + FGF2; Group 6 ¿ ventral root avulsion; Group 7- No lesion After 14 days of survival were analyzed (1) the expression of synaptophysin (synapses marker), GFAP (astrocyte marker) and Iba-1 (microglial reactions) by immunohistochemistry; (2) production of neurotrophic factors BDNF and the BDNF by hESC; (3) spinal motoneurons survival by Nissl staining technique and subsequent quantitative analysis by light microscopy; (4) by using qRT-PCR expression markers of the inflammatory response, including TNF-? and interleukins IL1?, IL-6, IL-10, as well as BDNF and GDNF factors. The results showed that treatment with ESC induced to overexpress FGF2 increased neuronal survival by 33%. Immunohistochemical evaluation with synaptophysin showed in groups II and V, treated with doxycycline + hESC and FGF2, respectively, a significant reduction of loss terminals in apposition to the cell body, reflecting a possible decrease in synaptic elimination process. In addition, there was also a significant decrease in anti-GFAP labeling (reactive astrogliosis) in the group treated with hESC + doxycycline compared to the other groups. An important finding was that hESC-GFP overexpressing FGF2, produced BDNF in the injection site of the cells and co-expressed GDNF in perimedullary region. However, cell therapy with hESC and FGF2 did not produce significant differences in the microglial reaction. The FGF2-treated group showed higher expression of ?2? and the group treated with hESC + doxycycline showed lower TNF-? expression and increased IL-10 expression, indicative of anti-inflammatory microenvironment. Taken together, the present results showed a neuroprotective effect of human embryonic stem cells modified to overexpress FGF2 as well as synaptic preservation and reduction of astrogliosis, can these results lead to promising regenerative studies aiming to improve motor recovery after brachial and lumbosacral plexus injuries Doutorado Biologia Celular Doutora em Biologia Celular e Estrutural 2013/23030-2 FAPESP
