Orientador: José Flávio Affonso de Almeida Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: Estudo in vitro dividido em duas partes: na primeira, foram avaliados porcentagem das paredes do canal onde houve penetração do curativo, máxima penetração do curativo, porcentagem da área penetrada pelo curativo e intensidade de fluoresceína no interior dos túbulos dentinários de 3 métodos de inserção do curativo. Na segunda parte, foram avaliados porcentagem de paredes do canal onde restou curativo, máxima profundidade de remoção do curativo, porcentagem da área de curativo restante e intensidade de fluoresceína no interior dos túbulos dentinários, entre 3 métodos de remoção do curativo. Foram usados 45 incisivos superiores humanos, para cada etapa, sendo que todas as amostras receberam o mesmo preparo químico-mecânico. Na primeira etapa, as amostras foram divididas em 3 grupos com 15 dentes cada: grupo A, o curativo de hidróxido de cálcio + clorexidina gel 2% foi inserido com uma lentulo em baixa rotação, grupo B, seringa Ultracal® e C, lentulo em baixa rotação + agitação ultrassônica (AU). Na segunda parte, os dentes também foram divididos em 3 grupos com 15 amostras cada: grupo 1, curativo removido com lima manual (#25) + soro fisiológico + EDTA 17%; grupo 2, soro fisiológico + EDTA 17% + Easy Clean® e grupo 3, soro fisiológico + EDTA 17% + AU. A seguir foram obtidos cortes nos terços cervical, médio e apical e as amostras resultantes foram preparadas para visualização no microscópio confocal com varredura à laser (MCVL). Para os métodos de inserção, os resultados mostraram que houve diferenças significativas entre os três grupos para a máxima penetração do curativo nos 3 terços. Na análise do perímetro do canal onde houve penetração do curativo, ocorreu significância estatística entre AU e os outros métodos de inserção também nos 3 terços. Para área de dentina penetrada pelo curativo, houve diferenças estatísticas nos terços cervical e médio entre os 3 métodos testados. No terço apical, só ocorreu diferença estatística entre o AU e os dois outros grupos. Para a intensidade de fluoresceína, não houve diferenças significativas entre os grupos em nenhum terço. Em todas as avaliações, AU obteve maiores valores. Na análise dos métodos de remoção do curativo, para o parâmetro porcentagem do perímetro onde restou curativo, houve diferenças significativas entre a lima e os outros dois grupos apenas no terço médio. Na máxima remoção de curativo, houve diferença significativa entre os 3 métodos de remoção nos terços cervical e apical, no terço médio AU e Easy Clean® não apresentaram diferenças significativas. Na área de dentina onde restou curativo e na intensidade de fluoresceína, houve diferenças estatisticamente significantes entre a lima e os outros grupos nos 3 terços. Pode-se concluir que a agitação ultrassônica do curativo foi o método mais eficaz para inserção de curativo nos túbulos dentinários, principalmente no terço apical, sendo sua superioridade diminuída no terço cervical. Para a remoção do curativo, AU e o Easy Clean® tiveram resultados semelhantes, sendo mais eficientes que o instrumento manual utilizado. A efetividade de remoção dos dois métodos foi menor no terço apical Abstract: An in vitro study, divided into two parts: in the first, the authors evaluated 3 methods for inserting the dressing, considering: the percentage of canal walls penetrated by the dressing; maximum penetration of the dressing; percentage of area penetrated by the dressing; and fluorescence intensity within the dentinal tubules. In the second part, 3 methods for removing the dressing were evaluated, considering: percentage of canal walls where dressing remained; maximum depth of dressing removal; percentage of area of remaining dressing, and the fluorescence intensity within the dentinal tubules. In the study, 45 human maxillary incisors were used for each stage; and all the samples received the same chemical mechanical preparation. In the first stage, the samples were divided into 3 groups with 15 teeth each: Group A, calcium hydroxide + % chlorhexidine gel dressing was inserted with a lentulo spiral at low speed; Group B, Ultracal Syringe®; and Group C, lentulo spiral at low speed + Ultrasonic Agitation (UA). In the second part, the teeth were also divided into 3 groups with 15 samples in each: Group 1, dressing removed with manual file (#25) + physiological solution + EDTA 17%; Group 2, physiological solution + EDTA 17% + Easy Clean®; and Group 3, physiological solution + EDTA 17% + UA. Subsequently, sections were obtained in the cervical, middle and apical thirds, and the resultant samples were prepared for visualization by means of a confocal laser scanning microscope (CLSM). For the methods of insertion, the results showed that there were significant differences among the three groups for maximum penetration of the dressing into the 3 thirds. In the analysis of the canal perimeter, where there was penetration of the dressing, there was statistical significance between AU and the other methods of insertion, and in the 3 thirds. For the area of dentin penetrated by the dressing, there were statistical differences in the cervical and middle thirds among the 3 methods tested. In the apicaI third, there was statistical difference only between AU and the other two groups. For the intensity of fluorescence, there were no significant differences between the Groups in any of the thirds. In all the evaluations, AU obtained the highest values. When analyzing the methods for removing the dressing, for the parameter percentage of the perimeter where dressing remained, there were significant differences between the file and the other two groups only in the middle third. For the parameter maximum removal of dressing, there was significant difference among the 3 methods of removal in the cervical and apical thirds. In the middle third, UA and Easy Clean® showed no significant differences. In the dentin area where dressing remained, and in the fluorescence intensity, there were statistically significant differences between the file and the other groups in the 3 thirds. It could be concluded that ultrasonic agitation of the dressing was the most efficient method for inserting the dressing into the dentinal tubules, particularly in the apical third, but its superiority diminished in the cervical third. For removing the dressing, UA and Easy Clean® showed similar results, and were more efficient than the manual instrument used.
