Benign prostatic hyperplasia (BPH) is an histopathological term for prostatic alterations represented by clinical, morphological and functional changes in the urinary tract. It is stablished that there is little correlation between patient's symptoms, gland size and urodynamics. Symptoms of prostatism are nonspecific and thus do not alone allow for diagnosis of obstruction. We consider necessary a complete medical history and a thorough physical examination including a digital rectal examination. Literature data were presented suggesting that the routine use of PSA in the evaluation of men with lower urinary tract symptoms. Preoperative selection of patients with BPH should include urodynamics studies. Finally, a little list of complex characteristics and manifestations of BPH was showed in this study with the hope that this information could be important to define the best treatment for the patient with lower urinary tract symptoms. It is reasonable to discuss the benefits and risks of the available treatment strategies with the patient and have the patient actively participate in the choice of therapy. 56 SPEC.
