Resumen
Wilson's disease consists in a inborn error of metabolism with marked neuropsychiatric manifestations. These manifestations need to be correctly identified and treated. There are four neuropsychiatric symptoms clusters established for Wilson's disease patients: behavior and/or personality disorders, mood disorders, cognitive deficits, and psychotic manifestations. Although such patients are more commonly seen in neurological and hepatological settings, mental health professionals must keep in mind a high level of suspicion, once first presentations may be of psychiatric nature. This study presents a review of literature about neuropsychiatric aspects of Wilson's disease, emphasizing diagnostic issues. It is presented the case of a 26 years old male patient initially with psychiatric polimorfic symptoms. The treatment of this patient with electroconvulsive therapy is presented, adding a report of this therapeutic modality, applied to that case, to the two only previous records in the literature. 61 3 B 876 880 Loudianos, G., Gitlin, J.D., Wilson's disease (2000) Seminars in Liver Disease, 20, pp. 353-364 Wilson, S.A.K., Progressive lenticular degeneration: A familial nervous system disease associated with drrosis of the liver (1912) Brain, 34, pp. 295-509 Shilsky, M., Wilson's disease: Genetic basis of copper toxicity and natural history (1996) Seminars in Liver Disease, 16, pp. 83-95 Sheinberg, J.H., Investigating diseases no-one's got (1983) N Engl J Med, 309, pp. 918-919 Akil, M., Brewer, J.G., Psychiatric and behavioral abnormalities in Wilson's disease (1995) Advances in Neurology, 65, pp. 171-178 Dening, T.R., Berrios, G.E., Wilson's Disease: Psychiatric symptoms in 195 cases (1989) Arch Gen Psychiatry, 46, pp. 1126-1134 Barbosa, E.R., Scaff, M., Cormelatti, L.R., Canelas, H.M., Degeneração hepatolenticular: Avaliação crítica dos critérios diagnósticos em 95 casos (1985) Arq Neuropsiquiatr, 43, pp. 234-242 Walshe, J.M., Yealland, M., Wilson's disease: The problems of delayed diagnosis (1992) J Neurol Neurosurg Psychiatry, 55, pp. 692-696 Ferenci, P., Gillian, T.C., Gitlin, J.D., An International symposium on Wilson's and Menke's diseases (1996) Hepatology, 24, pp. 952-958 Dening, T.R., Psychiatric aspects of Wilson's disease (1985) Br J Psychiatry, 147, pp. 677-682 Dening, T.R., Berrios, G.E., Wilson's disease: A prospective study of psychopathology in 31 cases (1989) Br J Psychiatry, 155, pp. 206-213 Davis, L.J., Goldstein, N.P., Psychologic investigation of Wilson's disease (1974) Mayo Clinic Proceedings, 49, pp. 409-411 Lang, C., Muller, D., Claus, D., Neuropsychological deficits in Wilson's disease (1986) J Clin Exp Psychol, 8, p. 149 Lang, C., Muller, D., Claus, D., Druschky, K.F., Neuropsychological findings in treated Wilson's disease (1990) Acta Neurol Scand, 81, pp. 75-81 Davison, K., Bagley, C.R., Schizophrenia-like psychosis associated with organic disease of the central nervous system: A review of the literature (1969) Current Problems in Neuropsychiatry: Schizophrenia, Epilepsy, the Temporal Lobe, 4, pp. 113-184. , Herrington RN (ed).
