Resumen
The pharmacotherapeutic follow-up service was implemented at the Hospital de la Brigada Galapagos Nº11-Riobamba. The study was applied in diabetic patients between 35 and 70 years of age; the recruitment was carried out in the outpatient area of the hospital, the patients were chosen under voluntary acceptance criteria. The research was carried out based on the DADER method, which involves a series of stages: the health status of the patients was analyzed through access to medical records; pharmacotherapeutic records were prepared for each patient, through interviews, surveys in which the needs related to medications were determined. Care plans were drawn up regarding the proper administration of medications, emphasizing the importance of compliance with pharmacotherapeutic therapy, proper nutrition, physical activity, among others. Monthly measurements of basal and postprandial glucose, glycosylated hemoglobin, cholesterol, HDL, LDL, triglycerides were taken, determining better control after pharmacotherapeutic follow-up, statistically evidenced by analysis of variance. In the study, 223 RNM were determined, of which 99 were due to quantitative ineffectiveness, 63 due to quantitative insecurity, 35 due to PSNT, 18 due to non-quantitative insecurity and finally, in minimal proportion, 5 due to unnecessary drug effect and 3 due to non-quantitative ineffectiveness; 261 PRM were also determined, of which drug-drug interactions predominated, determining 105, 51 due to non-compliance on the part of the patients, 35 due to PSNT, 18 due to AE, 17 due to inadequate dosage, regimen and/or duration, 14 due to PS affecting the treatment and, to a lesser extent, contraindications, administration, personal characteristics, among others, evidencing a notable decrease at the end of the study, obtaining 118 RNM and 188 PRM.
