Resumen
Objectives: In order to improve the initial unfavorable treatment results of mixed urinary incontinence (MUI) patients, we have designed a study which aims to identify selection criteria for initial treatment (surgical o medical) in those patients. The study would establish the success rates and the failure rates according to clinical, urodynamic features and quality of life score.
Methods: We have made an analytic and prospective study; all mixed urinary incontinence patients in the Hospital Universitario de la Samaritana were recruited.
Patients had to sign an informed consent before they were included. The initial treatment was carried out according to the attend physician criteria and Urologic Department guidelines. Initial clinical and urodynamic evaluation were done, and then the quality of life score was established using a questionnaire.Patients were followed with office visits to establish the quality of life score at 15 days after treatment, one month and three months.
Results: MUI patients who were surgical treated, achieved higher successful rates. Among those patients se successes were higher between those which have physical evidence of cystocele GII or greater and low VLPP values. The quality of life score improved with any treatment, particularly in surgical treated patients. Involuntary detrusor overactivity were not correlated with surgical or medical treatment success.
Conclusions: MUI patients who will benefit from the initial surgical treatment include women who have physical evidence of cystocele GII or greater and VLPP value below 60cmH2O. Involuntary detrusor overactivity evidence does not correlate with the final treatment success.
